Natural Medicine Business licensees are responsible for ensuring compliance with applicable statutes, the Natural Medicine Code at section 44-50-101 et seq., C.R.S., and the Natural Medicine Rules, at 1 CCR 213-1. The compliance tools on this page are not legal advice. The Division’s interest is in assisting licensees to establish and maintain compliant operations. Inventory Tracking

Required Forms for all Natural Medicine Businesses

Monthly Waste Tracking Form
The purpose of this form is to collect information to comply with Rule 3010(B)(5), which requires a Natural Medicine Business to maintain a Waste Log for two years as a business record. 
It is also required by Rule 3120, which details how to handle Natural Medicine Waste and requires a waste log. 

Transport Manifest
While transporting Regulated Natural Medicine and Regulated Natural Medicine Products, a Handler or Owner licensee needs to have a Transport Manifest, as required by Rule 3405(D). The downloadable and fillable PDF form linked above will allow you to fill in the necessary information to comply with the rule. 

Transfer Form
Regulated Natural Medicine and Regulated Natural Medicine Products may only be transferred between Natural Medicine Businesses' licensed premises. Both licensees must complete the NMD Transfer Form, which is linked above. 

Facilitator Request for Product Form
Transfers to Facilitators for Administration Sessions at Authorized Locations Other Than Healing Centers
Facilitators may only provide natural medicine services, including administration sessions, at authorized locations other than a licensed Healing Center as permitted under the Department of Regulatory Agencies rules. Facilitators should review DORA’s rules prior to requesting Regulated Natural Medicine or Regulated Natural Medicine Products from a licensed Natural Medicine Business.

A Facilitator may request Regulated Natural Medicine and Regulated Natural Medicine Products from a Natural Medicine Business for an administration session at an authorized location other than a healing center. The requirements for the request are found in Rules 5005(C) for Cultivations, 6005(E) for Product Manufacturers, and 8005(D) for Healing Centers and include:

  • The Facilitator’s Department of Regulatory Agencies issued license number;
  • The requested amount of Regulated Natural Medicine;
  • The number of Administration Sessions the Facilitator is requesting Regulated Natural Medicine for;
  • The number of Participants that will be consuming the requested Regulated Natural Medicine; and
  • The requested date for pick-up. 

Monthly Inventory Tracking Forms by License Type

Licensees are required to submit inventory tracking information to the Division at least monthly, and may submit inventory tracking information in real time as tracking events occur. Licensees must use the forms below to submit inventory tracking data to the Division, as required by rules. The video below these forms provides instructions on how to use them.

Cultivation Inventory Tracking Form
The purpose of this form is to collect information to comply with Rule 5025, Minimum Inventory Tracking Requirements. 

Product Manufacturer Inventory Tracking Form
The purpose of this form is to collect information to comply with Rule 6020, Minimum Inventory Tracking Requirements. 

Testing Facility Inventory Tracking Form
The purpose of this form is to collect information to comply with Rule 7035, Minimum Inventory Tracking Requirements.

These templates do not replace the requirements above to submit inventory tracking and transfer forms.
